The Community Navigator program originated with a team of formerly homeless young adults who came together to create holistic solutions. HopePHL works with Philadelphia’s Office of Homeless Services, Valley Youth House Synergy Project outreach team and Covenant House to offer the Community Navigator program.
HopePHL's Community Navigator program connects young adults, ages 18-24, many of whom have aged out of foster care, with vital resources such as shelter, employment, counseling, housing and coaching along with links to resources based on individual need.
The Community Navigators identify youth experiencing or at risk of homelessness, helps them navigate their exit from the child welfare system, and connects them with resources to secure stable housing, employment, wellness, financial coaching, and education.
