The Community Schools General Case Management program at HopePHL is a partnership between HopePHL, The Office of Children and Family Services and The School District of Philadelphia to offer support services to the community of Wright Elementary.
We partner with the school community to identify needs and provide resources and supports to students and their families through case management services, family forums and groups. |
